All previous diagnostic tests (X-Rays, MRI's)
Information about your past medical and/or surgical history

Follow-Up Appointment Visits
Please arrive 10 minutes prior to your appointment time. Arriving more than 30 minutes late could result in your appointment being moved to a "work-in" or rescheduled as we must respect those who arrive promptly for their appointment.
Please notify us 24 hours prior to your appointment if you need to cancel or reschedule your appointment by contacting our office at 718-698-3777 during normal business hours.
No Show Appointments
If you do not show up for your scheduled appointment, you may be charged a $25.00 cancellation fee that will be your responsibility (this fee cannot be submitted to your insurance company).
We accept most insurance policies and will verify benefits prior to your appointment. Be aware that some insurance require a co-payment and your referral at the time of your appointment. We recommend you contact your employer or individual insurance provider for any questions regarding insurance or benefit coverage.
It is our policy that payment is due at the time of service. Our office makes every attempt to verify your insurance benefits so that you will know what portion of your visit your insurance company is responsible for and what portion you will be responsible for, but we ask that you know and understand your own benefits to help ease the process.
